If no one here has a specific recommendation, you can probably find someone right for the job by contacting someone on the design staff of a high end furniture store in your area. Beverly Hall looks like it might be a good possibility; you can just call and ask to speak to a designer.
Most of them will have a good list of reputable providers for projects of all different sizes, even those to small to interest most general contractors, and they will only refer people they know to do good work. Be open with them about your project and budget. They will likely be happy to provide this information free to you because:
1. They will refer someone who pays them a referral fee. (This will not affect your price from the remodeler; it it just the way mutual referrals are handled in the home design business.)
2. They will hope that you remember them when and if you ever need to purchase furniture or pay for design assistance. (which is also the reason you can be assured of being referred to someone who will do a good and honest job for you.)
Don't call an actual design firm for this, because their designers work differently and will want you to pay for a consultation.
